  1. Transcript of Records — original and two(2) photocopies
  2. Certificate of Transfer Credentials or Honorable Dismissal (from the last school attended) — original copy
  3. Certificate of Employment (if employed) or Barangay Clearance (if unemployed) — original and one photocopy
  4. NSO issued Birth Certificate — two(2) photocopies
  5. NSO issued Marriage Certificate (for married women only) — two(2) photocopies
  6. Colored ID Pictures (size 1 x 1) — four(4) copies
  7. PRC License (if applicable) — two(2) photocopy
  8. Permit to Study * — two(2) photocopies;

Additional requirements for applicants who will take up Master of Arts in Nursing

  1. Related Learning Experience (RLE) — original/certified true copy and one photocopy
  2. Result of Board Examination — original and one photocopy

* Permit to Study is needed if the applicant is working in the Government or tuition being financed by the government or scholarship.

Enrollment Procedure

Graduate School Application

Step 1:
a. Go to Marketing & Student Recruitment/Admissions Office (MSRO/AO),Ground floor, Taft Wing
b. Admissions Office (MSRO/AO), Ground floor, Taft Wing Fill up an application for Admission Form and secure an Application Number.

Step 2:
a. Go to Respective Schools (Dean)
b. Present required admission documents
c. Get the signed Evaluation form (upon approval)

 

Graduate School Enrollment

Step 1:
a. Go to Graduate School 2nd Floor, Taft Wing
b. Present signed Evaluation form
c. Fill up Permanent Record Form
d. Fill up Adviser’s Slip with subjects to be taken

Step 2:
a. Go to Computer Laboratory 4 2nd Floor Mezzanine, Taft Wing
b. Submit Adviser’s Slip
c. Get printed assessment form

Step 3:
a. Go to Cashier’s Window 2nd floor, Nakpil Wing
b. Furnish signed assessment form
c. Confirm mode of payment
d. Pay due amount (including Php500.00 Profiling Fee)
e. Get validated Certificate of Enrollment form

Step 4:
a. Go to Respective Schools (Dean/Adviser)
b. Take Profiling Exam

Note: Signed Enrollment Assessment Form together with the Official Receipt of your Payment is your proof that you are officially enrolled. Upon enrollment, you will be requested to get an original copy of your Transcript of Records from your previous school attended bearing the remark “For Philippine Women’s University”
