Admission Requirements (Graduate School)

Present the following requirements:

• Transcript of Records - original and two(2) photocopies
• Certificate of Transfer Credentials or Honorable Dismissal (from the last school attended) - original copy
• Certificate of Employment (if employed) - original and one photocopy
• Barangay Clearance (if unemployed) - original and one photocopy
• NSO issued Birth Certificate - two(2) photocopies
• NSO issued Marriage Certificate (for married women only) - two(2) photocopies
• Colored ID Pictures (size 1 x 1) - four(4) copies
• PRC License (if applicable - one photocopy)
• Permit to Study * - two(2) photocopies;

* Permit to Study is needed if the applicant is woriking in the Government or tuition being financed by the government/scholarship.

Additional requirements for applicants who will take up Master of Arts in Nursing

• Related Learning Experience (RLE) (original/certified true copy and one photocopy)
• Result of Board Examination (original and one photocopy)

2	Respective Schools (Dean)	Checking and Evaluation Present required admission documents Get the signed Evaluation form (upon approval)

Notes: Signed Enrollment Assessment Form together with the Official Receipt of your Payment is your proof that you are officially enrolled. Upon enrollment, you will be requested to get an original copy of your Transcript of Records from your previous school attended bearing the remark "For Philippine Women's University"

2	Computer Laboratory 4 2nd Floor Mezzanine, Taft Wing	Registration Submit Adviser's Slip Get printed assessment form
3	Cashier's Window 2nd floor, Nakpil Wing	Payment Furnish signed assessment form Confirm mode of payment Pay due amount (including Php500.00 Profiling Fee) Get validated Certificate of Enrollment form
