High School Graduate
- Form 137 (original and three(3) photocopies)
- Form 138 (original and three(3) photocopies)
- Certificate of Good Mortal Character (original and three(3) photocopies)
- Birth Certificate (original and three(3) photocopies)
- Long brown envelop (five(5) pieces)
- Colored ID picture with maroon or red background (size 1 x 1 - four(4) pieces)
Transfer Students
- Honorable Dismissal (original and three(3) photocopies)
- True copy of Grades (original and three(3) photocopies)
- Transcript of Records (original and three(3) photocopies)
- Certificate of Good Mortal Character (original and three(3) photocopies)
- Birth Certificate (original and three(3) photocopies)
- Long brown envelop (five(5) pieces)
- Colored ID picture with maroon or red background (size 1 x 1 - four(4) pieces)
Caregiver Course Requirements
- Certificate of Good Mortal Character or Barangay Clearance (original and three(3) photocopies)
- Transcript of Records - with at least 72 units in College (original and three(3) photocopies)
- Marriage Certificate - for married women only (original and three(3) photocopies)
- Birth Certificate (original and three(3) photocopies)
- Long brown envelop (five(5) pieces)
- Colored ID picture with maroon or red background (size 1 x 1 - four(4) pieces)
Non-High School Graduate
- Waiver for High School graduate
- Form 137
