• High School Card - Form 138
• Letter of Recommendation / Certificate of Good Moral Character from High School Principal / Guidance Counselor (original copy)
• NSO issued Birth Certificate (1 photocopy)
• Marriage Certificate (for married female students) (2 photocopies)
• ID Pictures (1.5 x 1.5) (2 copies)

HOW TO APPLY

• Get and fill-up an Application for Admission Form and secure an application number at the Admissions Office (AO).
• Pay the Php 500.00 Testing Fee at the Cashier’s Window
  • Note: Free Testing Fee for Valedictorians / Salutatorians. Graduates of PWU High School and JASMS Manila/QC are exempt from taking the entrance exam
• Take exam on your scheduled date at Seminar Room 7/8
  • Test Results will be forwarded directly to Dean/Director for Admission on the same day
  • Music/IFAD students proceed to take the Music Audition / Art Test at the School of Music and Fine Arts respectively

HOW TO ENROLL

• Get acceptance letter and fill up Permanent Record Form (PRF) at the Seminar Room 1
• Pur PRF and required admission documents in brown envelop and present to Admission Director for checking
• Get Adviser's slip and subjects to be taken
• Submit admission documents and PRF at the Seminar Room 2
• Present Adviser's Slip for issuance of Student Number, Encoding of Subjects, and Assessment of Fees
• Get assessment print-out
• Pay Assessed Fee. Get OR at the Cashier's Windows
  • Note: Student may opt to initially pay the Reservation Fee of Php 5,000. This fee is non-refundable but will be applied to tuition payment upon enrollment. However, the total amount due based selected mode of payment must be paid before the first day of classes otherwise the Certificate of Enrollment Form will not be released.
• Present OR, Adviser's Slip, and Assessment Printout at the Assessment Window, Finance Office
• Sign and get validated Certificate of Enrollment Form

AFTER ENROLLMENT

• Get Library Card at the Seminar Room 1
• Secure your ID at the Multimedia Office
• Have Physical/Dental Examination at the Medical/Dental Clinic
• Purchase Uniforms and other school supplies at the Bookstore
